Student-athletes are required to have a current OHSAA Physical Form on file in the athletic office to participate in extracurricular activities.

Any student-athlete that is removed from a contest due to a suspected or diagnosed concussion must have a physician fill out and sign this form to gain clearance to return to participation.

Students who complete a full season of a Special Olympics sport can earn a Varsity Letter “S” that they may choose to put on a letter jacket. Letters are available to students in grades 9-12 and Salem Athletics will provide the Varsity Letter “S”. After completion of the form by the parent/guardian and Coach, please submit the form to the athletic department.
